A transcript is a comprehensive record of your academic progress, including transfer courses, credit earned by exam, and earned degrees.

Your name appears on your transcript as it appears on your university record. Official transcripts include the university seal and the signature of the Registrar.
Student academic records are classified as confidential and may be released only with the student's authorization, in accordance with the Public Information Act and the Family Educational Rights and Privacy Act of 1974.
Transcript Order Information
Official transcripts can be requested in three ways: Online, by mail, or in person (we do not accept transcript requests via fax). If the only courses you have taken at USU are still in progress, you will not be able to request a transcript. You will need to request an Enrollment Verification.
Ordering an Official Transcript Online (Lowest Cost Option)
Official transcripts can be ordered online through the National Student Clearinghouse by visiting www.getmytranscript.com.
You can also order your transcript from the National Student Clearinghouse by logging into ACCESS using your A-Number and password. Once logged onto ACCESS select 'Student' and then 'Student Records' and then 'Order Transcript'. You will then be directed to the Clearinghouse at that point to complete your order.
All major credit cards are now accepted for online transcript orders.
Credit cards are not accepted for transcript requests by mail, in person, or fax.

If you last attended Utah State University prior to 1980 or you do not have a present student account at USU you may order your transcript directly from the National Student Clearinghouse at http://studentclearinghouse.org/

Ordering an Official Transcript by Mail
Official transcripts can be ordered by mail by sending in a completed Official Transcript Request Form. Please include the payments as noted on the form.
Cash and check are both acceptable forms of payments. Credit cards are not accepted for requests in person, by mail, or fax.

If you do not have access to the Official Transcript Request Form please send a written request with the following information to the address above:
- Full name and any previous last names
- Phone number
- Student ID (A-Number)
- Birth date
- Complete address to send the transcript to
- Check payment ($5.00 per mailed transcript and $8.00 per faxed transcript)
Official transcripts can be requested in person at the Registrar’s Office, Taggart Student Center room 246. Picture ID will need to be shown at the time the request is made. Transcripts can be paid for with cash or check and will be released to the student at the time the request is made. Individuals other than the student requesting the transcript must have written documentation with a signature releasing their transcripts to the individual requesting it.
An unofficial transcript lists all of a student's coursework but does not include the university seal, the signature of the Registrar, or the name of the university. The Registrar’s Office does not print unofficial transcripts. Unofficial transcripts are available online via the ACCESS student records system.
